Corrections and source concerns

How to report an error, missing context, accessibility problem, source update, retraction, or rights concern.

What to send

Use the contact page and identify the page URL, the statement or metadata field at issue, and a reliable source supporting the correction. Do not send personal health information.

  • Broken or incorrect source link
  • Retraction, withdrawal, correction, or superseded guidance
  • Misstated title, author, journal, date, country, or identifier
  • Clinical-safety, copyright, privacy, or accessibility concern

What happens next

A credible safety, retraction, rights, or identity concern can place a page on hold while it is checked. Minor metadata changes may be corrected directly; substantive interpretation changes require editorial review.

  • The source of truth is updated before regenerated HTML.
  • Modified and review dates are not fabricated.
  • A correction does not erase the original source trail.
